Health Promotion to Hypertensive and Diabetic Patients in a Family Health Team: an experience report

Hypertension and Diabetes stand out among the main public health problems today, for its recognized prevalence and associated risk factors. With expressive indicators, are the leading cause of mortality and morbidity, impacting the lives of individuals, as well as straining health systems. In view of the difficulties encountered in monitoring this public, was designed by a Family Health Team in the city of Petrolina – PE, extramural health care activities with the aim of improve the follow-up of patients with these diagnoses. It represents a qualitative, descriptive study, of the experience report type, on the actions of prevention, promotion and health treatment developed. The methodology used focused on extramural care and workshops in group dynamics. With the change of look at care for this public, there was a positive effect on the team's indicators: decrease in patients with a decompensated health condition; performing risk stratification in order to reduce associated complications; improving the accessibility of patients to the health service; patients' commitment to self-care in health and the strengthening of the bond between the health team and community through the use of active methodologies in health promotion.
